4. Interested subscribers must submit the following documents to avail of the offer:
   • POID of the business owner - this may be waived as long as the standard authentication process as defined in the Customer Authentication Policy is done by the frontliner/salesperson processing the application to ensure that it is the SOR who is transacting with us.
   • Signed copy of the subscriber contract or any documentation that’ll show subs agree to the terms and conditions.
